Virtual Accuplacer Request

Complete all areas of the form and submit, all areas must be completed in order for your submission to be processed. You must use your Rhodes State ID number, if you do not have a number, please call the Testing Center at 419-995-8476 for instructions. Please allow three business days for processing. The voucher will be emailed to you once processed. Examity, the tool in which you will be proctored, charges a $25 proctoring fee for each test session. Before submitting please make sure you have a microphone and webcam (built-in or external). If you need further assistance, please contact the Testing Center at testcenter@rhodesstate.edu or 419-995-8476. Your voucher will be good for 30 days.
